Initial Certification Requirements

  1. Application: Form 644
  2. Identification: 2" x 2" current photo
  3. Fingerprints Cards: Submit the fingerprint verification form issued by an approved Fingerprint Vendor (Nevada approved vendor list - Form 619). Fingerprints expire after two (2) months.
  4. Fee: $250.00
